First Time Patient?

Please remember to bring the packet with you to your appointment and to fill out the information before you arrive at our office. Please arrive 15 minutes before your appointment time so we may be able to complete the registration process. If you are more than 15 minutes late for your appointment, you will need to call to reschedule or see if the physicain or audiologists can still fit you in that day. The patient information forms for you to fill out and a map are available on the link above. If your insurance requires a co-payment or deductible, it will be due at time of service. It is your responsibility to know the amount of your co-pay and/or deductible.
